Admission, Discharge, and Transfer Event Notifications
CMS is modifying Conditions of Participation (CoPs) to require hospitals, including psychiatric hospitals and CAHs, to send electronic patient event notifications of a patient’s admission, discharge, and/or transfer to another healthcare facility or to another community provider or practitioner. This will improve care coordination by allowing a receiving provider, facility, or practitioner to reach out to the patient and deliver appropriate follow-up care in a timely manner. This policy will be applicable 6 months after publication of this rule.

Koble-MN connected to eHealth Exchange
Did you know that Koble-MN HIE is an eHealth Exchange Two-Way Responder? This means that authorized users of the Koble-MN HIE, with written consent from their patient, can now query the eHealth Exchange (which includes CommonWell and Carequality for Koble-MN Participants) for additional clinical documents that pertain to their patients from other participants of the eHealth Exchange through their login with the Koble-MN HIE.
In Minnesota, this means that key information from the large health systems is now accessible, as well information from other Health Information Exchanges and federal agencies from across the U.S.
